The Five Case-Mix Adjusted Components

The five case-mix adjusted components are integral to CMS Patient Driven Payment Method calculations.  

Physical Therapy (PT) - PT focuses on maintaining, promoting, or restoring lost physical function following a serious injury or illness.

Both the PT and OT components use the clinical reason for the SNF stay and the resident's functional status to assign a patient to one of 16 CMGs.

Occupational Therapy (OT) - OT focuses on treatments/interventions to increase patient's safety and independence with their desired activities of daily living (ADLs) and functional mobility.

Speech-Language Pathology (SLP) - SLP specializes in the evaluation, diagnosis, and treatment of communication disorders (speech disorders and language disorders), cognitive-communication disorders, voice disorders, and swallowing disorders. 

The SLP component uses the patient’s PDPM clinical category, cognitive function, the presence of an SLP related comorbidity, and the presence of a swallowing disorder or a mechanically altered diet to assign a resident to an SLP component group.

Non-Therapy Ancillary (NTA) - NTA are a list of diagnostic categories and clinical support services needed to care for those patients that may be more medically complex or demonstrate a higher acuity. NTA’s are calculated, upon admission, by assessing a patient’s clinical profile.

Nursing - The nursing component under PDPM uses the same basic structure as used under RUG-IV for assigning patients to a nursing group. The nursing case mix component utilizes the use of extensive services, certain clinical conditions, the presence of depression, restorative nursing services provided, and the patient’s functional score to assign a patient to a nursing case-mix group.
