The Family Review screen is designed to share information about a family member’s current risk and to assure them that care will be based on the goals and expectations of their loved one. It includes a visual representation of the patient’s current health status, accompanied by structured text about that risk. This prepares the family for the possibility of an untoward outcome, resulting from the degree of frailty present.
